Going home after hospital discharge: a practical checklist

Discharge day is often busier than expected: prescriptions, reports, belongings and the question of how to get home. This short list is meant to make leaving the hospital calmer.
Before you leave
- Make sure you have every document, including prescriptions and the discharge summary
- Save the date of your follow-up appointment on your phone
- Decide whether you will pick up medicine from the on-duty pharmacy
Arrange the ride in advance
Rather than waiting at the door with a tired patient, book the taxi ahead. Send us the discharge time and the number of passengers on WhatsApp and our car will be waiting at the hospital exit.
During the ride
Take your time getting into the car. If you tell the driver about any mobility limits, they will drive calmly and carefully.
Arriving home
If you need to stop at a pharmacy, mention it during the ride and we will plan the route around it.
